1. Liability Release: In signing this agreement, I hereby release Mid-South Walleyes, their staff and members, tournament sponsors and their employees and the Marina of departure from any and all damages, claims, demands, costs or expenses relating to injury of any person's property which I may cause by reason of participating in or in connection with this tournament.
2. I further consent and agree to grant Mid-South Walleyes, the Marina of departure, and all sponsors full use of any name, still pictures, video likeness and background information in connection with Mid-South Walleyes, the Marina of departure, and all sponsors regarding articles, newscasts and press releases. I further understand that I will not be entitled to receive monies, royalties or other compensation from such publications.
3. I signify by my signature that I have read and understand the agreements 1 & 2 above. Both team members must sign this form to be considered for the earliest postmark for departure order.
